Hmmmm interesting question, I don't think I can ay I've impulse bought many games since 1998 when I got broadband.

If Mario 64 counts, going with Mario 64, a little easy but yes, I never saw anything on it before I got it. Of course I'd played previous Marios, hah well in that case I'll say Super Mario Bros.

Mortal Kombat versus DC is always my worst buy thing, but then again I had seen a video or two. Hmmm, Radiata Stories was a pretty bland one I picked up without having seen anything on it I think.

Worst: Final Fantasy 8 (I was shocked at how awful this was considering how much I loved FF7 and FF9. My girlfriend at the time told me to avoid this one and just play 7 and 9 but I decided to buy it anyways. I should have listened to her.)

Worst: Mario Power Tennis with new play control for the wii. I liked the one on the gamecube and thought I could play without the motion controls as well. I was wrong and the controls sucked bad, you couldn't do anything right.

I only had 2 impulse buy since I tend to look at my games and it was the best game I bought and probably the worst game I bought (returned it like 2 weeks after).
