They rushed everything about it and served a half-baked game dripping off the plate to a fan base starving for quality Fallout content. They wanted to make a MMORPG with little to no experience with online play. It's no surprise the game has been performing so poorly. Hopefully they learn from this and it humbles Bethesda.
