Did the first Diablo have the colors? I remember it had the various adjectives that added to the items power, but don't remember any rarity colors.

I'm not sure if the original Diablo had colours outside of quest items, but Diablo 2 was definitely one so that's the latest possible.

Diablo was built as a realtime Angband clone, so maybe we can start there.

As an aside: Castle of the Winds did have colour based rarity, but it was very simplistic.

Yes. It had white, blue, yellow, and like brown as I recall. Or maybe it was just yellow for the third one. Either way it definitely had at least three colors.

If that means just that the strength of items is denoted by color, the earliest game with that basic concept I've played is Might and Magic III from 1991, where what the item is made of detemines its strength -- wooden<leather<brass<bronze<iron etc.

I guess it's close, but really that's more like a prefix system. this would be if an iron cuirass could be a common/rare/epic iron cuirass. tiers are like an item-wide % improvement. prefixes are more of a straight +X Stat increase which is what you get in M&M3.
