I think I figured out the confusion

1. I thought "i don't pay to go to the doctor" means he doesn't go to the doctor, but it actually means that when he does go to the doctor, he doesn't have to pay
2. "my question still stands even if you weren't talking about obamacare" meant that even though he had an obamacare question, it was still valid, but I thought it meant his question wasn't specifically about obamacare, which it was
