What exactly is the difference between a trans woman (before the operation) and an effeminate gay man?

Well, for one, a transperson doesn't necessarily have to be either heterosexual or homosexual in order to qualify. More importantly, if the effeminate gay man readily identifies as a man, then he is not transgender. Meanwhile, in transgender individuals, it is quite literally impossible for them to consider themselves the same gender as the one they were assigned at birth based on their sex. Their brain will not allow it.

People say that until someone flipflops on what the identify as.

Some people are mistaken. Some people play up an identity for attention. This doesn't stop transgender from being a real thing. In a world where people incorrectly self-diagnose themselves all the time, this cannot be used to delegitimatize all transgender individuals.

Funny people bring up D&D, pretending, LARPing, whatever.

The gaming community has a fair number of transgender people, which in the end, only makes sense. We're a community where we assume identities that we couldn't normally be. I've met dozens going to conventions. You'd think we as a community would "get it" a little more.

Everybody here should understand what being transgender is like because we play video games?

That'd be nice, but I'm speaking less from an absolute sense and more from a relative sense.

But then again maybe I am seeing it. Maybe it'd be even worse in other communities than it is here on CE for accepting trans people.

A trans womans groin is surgically made. Wouldnt that make it pretty delicate? Like, any accidental knee there could cause serious, if not life threatening injury?
