For a squib safety story, Stallone almost got his fingers blown off on the set of First Blood because in the scene where they are shooting at him when he runs into the cave he puts his hand on something and didn't realize that a squib was set there and set to go off. He felt it blow up as he was removing his hand from it and finished the scene running into the cave.
