I'm okay when it's the occasional bite, sometimes things slip through and it just didn't get butchered/cut perfectly. You can toughen chicken by cooking it too quickly, but most of the time it's because it's been bred badly. I've never experienced it in the US (haven't lived there in over 10 years though) but I get it all the time here in New Zealand.
