Knowing how all variations of the character work you can surmise he went to Arkham for psychiatric treatment pretty much immediately after his arrest. We already know Harleen's story across multiple variations of the character and it's pretty easy to see how A: Joker could waltz right into that story from its ending without the need for Arthur to have established a career as a notorious criminal mastermind, and B: how that story could, playing out pretty much to the letter how it does in nearly every adaptation, be well adapted to the overall aesthetic and style of the previous film. Casting seems on point, Lady Gaga has been establishing herself more and more in film and imagining her playing a crazy/sadistic clown is not far fetched at all.
