But this one had a title that was really weird. It was something like

The chance of defects aren't significantly greater than you see from older parents (and by "older", I mean 40 instead of 20).

It's not really incest that's the problem, it's inbreeding. One incident of incest isn't likely to result in a weird mutant baby, it's constant incest over generations that lead to the detrimental effects.

one generation of incest is bad as well. recessive mutations that lead to deleterious phenotypes are much more likely to be carried by both siblings than by two strangers, meaning it is significantly more likely that their child will inherit both recessive copies and express it

It's both. Inbreeding relatively distant relatives will lead to detrimental effects many generations later, but the closer they get, the less iterations it takes, with immediate family bringing about immediate problems.
