Quickbreads (which includes muffins) generally differ from cakes (including cupcakes) in a number of ways, most notably texture. You generally get a denser crumb with muffins than with cupcakes, especially if you're comparing the muffin/wet-dry method (liquid fat, mix wet ingredients and dry ingredients separately, then combine) to the creaming method (solid fat, cream together with sugar before adding wet ingredients, then add dry), which is another way the two typically differ. Exceptions exist ("cake" is a very broad term that does actually include some quickbreads, such as some carrot cakes that are virtually indistinguishable from muffins), but that's the general trend. It's more than just whether or not it's frosted.

Proportionally speaking, muffins actually often have more sugar than cupcakes, especially if you're looking at commercial products. You can make savoury muffins, but that's not really the norm (that niche is more commonly filled by scones/biscuits, which are another beast entirely).
