I can't think of many examples, except for Super Mario World, where the main theme, athletic theme, underwater, ghost house, bonus stage and even the castle theme were all based on the same basic melody. Depends just how much it gets used. SMW's a particularly egregious example, where as soon as I noticed I was kind of annoyed by how overwhelmingly derivative the soundtrack was, even though it was still pleasant to listen to. If it just comes back from time to time to tie together common thematic elements, though, I could see that being fun (not that I can think of any particular examples of that on a whole-game scale).
I guess it's kind of like a common theme in a symphony. When it appears in a few different contexts and is used to tie everything together, that's fun. When it's just repeated over and over again with mild variation (see: Bolero), you've gotta do something to keep things interesting, otherwise it's just awful (see: any performance of Bolero that is even a little bit sub-awesome). --- This is my signature. Variations are fine by me. What bothers me is hearing the exact same song over and over again in a game.

I like it when a certain motif makes brief appearances throughout multiple tracks, like SMG's main theme being in a part of Gusty Garden Galaxy.

I like when one or several main themes get variations throughout the soundtrack. That's usually pretty cool, especially when the melody is the same but due to changes in speed or accompanying instruments or such, the tone is completely different.
But should an entire soundtrack be one song remixed a whole bunch? No. Not even for old school short games w/ a very small soundtrack. Hard to think of video game examples at the moment... Baten Kaitos soundtracks tend to use similar motifs in a bunch of songs, for one... I usually find it more in anime. Ronin Warriors uses several instrumental variations of each of its three main songs (Samurai Heart, Stardust Eyes, and Faraway) and they're all great. Cutie Honey series always uses a ton of variations of the theme song in the soundtrack to good effect, too.
