Probably just as well, my apartment complex doesn't put doorbells on the front of the buildings so it'd be a pain to know when the delivery guy got here.
So anyway I just answered based on what I used to do at my old apartment when I got food delivery like 3 or 4 days a week.

That's nice in theory but how often do you actually do it?
Like, when I go to a restaurant, all I really want from a waiter is to be available in a reasonable amount of time and to not mess up the orders. And like 98% of them do just that. So I give them a standard 10-15%. It's not like I'm rating every waitress on a scale of 1-10 on a variety of different grading criteria. This isn't a video game review here.
Also, it doesn't really give you control over the type of service, since you do it afterwards. If you were required to tip as soon as you sat down, THAT would probably dictate the level of service you would receive. That also is entirely subjective. Some rich guy comes in and muddies the whole thing by showering them ridiculously.
as a former delivery driver, I assure you guys that 95% of any mistakes is cause of the employees at the store, not the drivers themselves
I remember I actually arrived late once cause I had to get gas. that was pretty much the only time I actually caused a tardy. one time out of hundreds of deliveries
and yea, if you consistently be nice to drivers, you'll gain a rep that you're a good customer, and eventually we'll start hooking you up with stuff
extra bread sticks, soda, hell, in one occasion, we even upgraded the size of the pizza for them

This weekend I stayed in a hotel in NJ, and the cleaning staffed knocked on my door because they wanted to see if they could clean my room. It was 2 hours before checkout, I wasn't ready, I was watching my baby, and my wife was in the shower. I thought it was extremely rude and lazy so I didn't leave them a tip, just a note. Maybe I didn't get to dictate what my service was, but I got to dictate what service I paid for. Or what I'd pay for that service - same thing, to me.

Maybe I've just been lucky, but screwed up orders are very rare for me.
Like, for me, a tip is just an annoying exercize in extra math I'm forced to go through at the end of the night. I can probably count on one hand the times I've left no (or a really small) tip due to terrible service. I've NEVER left an extra large one for great service, because I'm not even sure what "great service" is. Just bring me the food I asked for, show up every five minutes or so to fill up the water, and leave us alone, that's all I really want. No way to really go "above and beyond" that imho.
What I especially hate is when the manager comes over just to say hi. I can't even lower the waitress tip in response to this. :(

I usually just give 2 bucks. I am cheap. Then again, I only get delivery about once a year, usually when I am too lazy to leave the house. Almost all my pizza orders are for pick up, usually because I am close enough to get it myself, or I am on the way that way anyways.

Yeah I felt the same way, but they barely speak English.
I used to not tip, but then I lived with people who delivered food and dated people who served food. I still don't entirely agree with the principle, but I'll usually tip $2+ dollars nowadays.
I also tipped my long-time barber $6.50 yesterday, but that was more because it's the last haircut I expect from her before I move out of town.

So you want to tip the waitress less for something the manager does...? to answer the latter - no. second sentence was meant to say that per my tipping philosophy, I'm not going to tip the waitress less for something the manager did.
to answer the former - they will barge into the room if you don't answer the door. and yes I had a "privacy please" tag on the doorknob, perhaps I neglected to mention that.

I like to pleasantly surprise people with relatively low-paying jobs. Yesterday I ordered pizza for the first time in like two years, actually, and since it was on time and correct, I gave the guy ten bucks.
If things were messed up or it was ridiculously late I would have gone down to about three.
