The people who originally drafted up the Constitution (as well as those who amended it over the last couple centuries) didn't think we'd ever be in a position where something that stupid could ever happen. By the time it became plausible that it could happen, Republican obstructionism and control over several state governments made it effectively impossible to amend the Constitution to patch that obvious hole.

Don't forget Eugene V. Debs. He ran from prison last century and got a million votes or so. He wasn't in prison for a just reason, though.

No, that wouldn't happen automatically. The 25th amendment allows the president to voluntarily transfer presidential authority to the VP or for the VP and the majority of the cabinet to declare the president unable to fulfill his duties, so the VP would act as president in those cases. Congress could also impeach and remove the president which would mean the VP becomes president.

If none of those things happen, then it's uncharted legal waters.
