If they refuse to cover something that they clearly should, contact your state's department of insurance.

I did that when my insurance company refused to pay for one of my doctors because he was "out of network", even though I sent them a screenshot of their own website showing he was definitely IN network.

I don't get it either.
A minimum claim amount sure. Like car insurance with $500 deductible, if you have less than $500 in damage, you should pay the difference in deductible. There's a ton of work involved with an insurance claim.
So far the waiver when not at fault is the best we've got.

Pretty much everything with any insurance industry is fucking terrible. You can specify one, but they're all objectively horrible.

That's the thing that bugs me the most about US health care: If it were a matter of the government spending very little money on health care and leaving everyone to fend for themselves, and that's why availability was so terrible, I'd disagree with that as a matter of moral principle, but it'd at least make sense and could be justified under a different moral framework from mine. But no, the government spends twice as much per capita as anyone else (which is a lot when you've got that many capitas) and has jack shit to show for it. It's a waste of money and they fail to provide adequate care, which is just lose-lose all around and absolutely nobody should be okay with that regardless of where they fall on the political spectrum.

Not paying out is literally how they make money. The more customers who don't use their insurance, or they can deny coverage for, the more money they make.
