I think the crucial think you're forgetting is that ninjutsu, or genjustsu in this case, is magic, so our understanding of laws and logic do not apply. Though now I am thinking of some sort of rotation where people spend half the day awake in the real world and the other half in some endorphin-laced dream and would that lead to better attitudes across the real world? Or would those people perhaps be embittered they can't remain in the fantasy and lash out against the real world with increased violence.

It's specified that even being indoors or in a cave won't save you from the effects. The only thing that kept a few people safe was Sasuke's Rinnegan-powered Susano'o.

I imagine that if it permeates through caves and such, it would also work along the curves of the planet. The other aspect is that it grows a tree that pulls everybody into its roots/branches.

First how do you know that naruto world isn't flat? The maps shown in Naruto don't even show the whole world also it works due to basically magic has people in caves and inside builds are also affected

Fictional world doesnt mean anything goes, dont think. Internal logic has to hold up, so yes, if something is said to reflect off the moon and affect the whole world we are right to remind the writer that the moon isnt visible to the entire world at any given time.

Unless in-story its specified that the world is indeed flat (not the case) or that the light is bouncing off mirrors to travel around the world (also not the case).

There are cases where a story can rightly ignore real world logic and the answer really is its magic. Like why Amaterasus flames are so intense yet dont burn someone unless they make direct contact. This isnt one of those cases.
