The developers mustve had something in mind, for putting this in the game. But for the life of me, I dont know what the hell it could possibly be perceived as other than a BJ.

Although the boxer is usually sitting it is quite common for cornermen to pull out the elastic on the pair of shorts while in the corner on the stool to allow the boxer to be able to breathe better.

You also commonly see them squirt water in their crotch. Besides the water on the top of the head this is the best area to squirt water as it hits the inside of the thighs and the femoral artery and gives the fighter a jolt of alertness along with greatly cooling him down.

These are the only two legitimate things that I could see going on but obviously the animation doesn't match up with that.

The NES game is an attempt at a direct port of the arcade game. The arcade game had tough looking boxers with over the top silly, cartoony graphics and gameplay. For example, flexing an oversized beefy arm when you win, and punching your opponent so hard he flies out of the ring and hits the ceiling.
In between rounds, the arcade game developers wanted to express that the boxers are getting beat up and are struggling.
What you see in the arcade version is an animation of the boxers' faces looking wrecked and on the verge of defeat. The assistant, in an attempt to tell the boxer to keep at it and get back into the ring, grabs the boxer by his clothes and attempts to pull him back into the fight in an overly dramatic cartoon animation. The only clothes the boxer is wearing is his shorts, so he's grabbing him by the waistband.

For the NES port, the developers has fewer colors, fewer frames, and smaller sprites to work with. The result is boxers with much less expressive faces, and assistants who remain much closer between the boxers' legs.

I didn't word it the best. The shorts are elastic, not tied with a string or anything like that. In order to help the fighter breathe better you often see in boxing the cornerman will hook his hands inside the shorts and pull them out thus allowing a fighter to catch his breathe easier.

But it appears the previous poster has the answer why the animation is the way it is.

Oh, alright.

I read it like pulling out the elastic entirely.

Seemed like it would be difficult to get the pants back on for the next round.
