They're usually assigned to flights with people on a watch list - no flys can't fly period - or an associate or family member of these people. Certain 'hot routes' (L.A., NYC, DC, London, Tokyo, etc.) will have a team randomly assigned. That algorithm is classified.

ive seen the cockpit door open in like half of the flights Ive been on since 9/11. If someone in the cock pit needs to use the restroom, they have a flight attendant stand in front of hallway into the first class restroom. The person needing to use the restroom does so, they knock on cabin, get let back in, flight attendant leaves hallway.
