If someone comes to their appointment drunk as hell or misses their appointment because they were arrested for reckless driving they porbably won't get it either. Organ supply is much less then demand. They aren't gonna waste it on someone who will throw their life away. Also immunosupressents are often used after the transplants to decrease the chances of your immune system rejecting the organ. Can't forget that the surgery involves a lot of blood, so that's potentially wasted donor blood as well as putting the surgical team at an increased risk of contracting covid by performing the procedure.
