I basically brought them into the office and talked through the situation that I was firing them for, and once we talked through it I say something like "And you understand why that's unacceptable, and the reason I'm going to have to let you go?" It generally works really well and doesn't start things off instantly hostile. I forgot to answer one of your questions. The guy had the arm holding the phone grasped by one agent. The other agent took the phone from his hand and hung it up. Someone asked "what do we tell the caller if s/he calls back?".
