They get the visa before coming to the US. Once in the US, they have 90 days to get married. Once married they file for an adjustment of residence status. Once that's approved (can take many months), they get a temporary 2 year Permanent Resident status (the green card). Getting divorced during any of that can raise red flags and cause USCIS to scrutinize the marriage. I will say that for any "older" (like... 30+) woman from Ukraine, Russia, Belarus, etc., getting married can be a very difficult prospect.
