It's fast-paced, it's violent, but the difficulty is mainly because health packs are rare to find and if you do find them they only fill up small amounts of your health. And it's a roughlike, so if you die you go all the way back to the start. They kept marketing it as a throwback 90's shooter, but if you try to play this like Doom or even Dusk, you'll get your shit pushed in.
