Examples:

illusion of Gaia. To get the secret boss and the real ending, you had to collect all the hidden gems. The first gem was in the first town, which you can not return to once you left. You can return to any other area except this one.

Your only choice was to restart your 20 hour game again to get that one freaking gem for the actual ending.

Super Mario RPG: Square/Squaresoft, the kings of "SCREW YOU" gameplay. I expect lots of people to post an example from every Final Fantasy, but let's rant about Mario RPG.

During the game you are told about hidden chests. Unfortunately they have moon logic and you will rarely find any on your own until you get the item that gives a tone whenever you enter an area with a hidden chest, narrowing down your search.

In the first castle of the game, you hear that tone. You try to jump everywhere, but nothing. Well it turns out the tone should have said "oh by the way, 30 hours ago you should have jumped on Toad's head, and he only stands there for a second, so you should have been quick. Oh and start a new game, jerk". -_-

Crackdown: The game that only made a profit because it had the Halo 3 Beta. You had to collect hundreds of orbs, and like Grand Theft Auto, you have NO indication of what you already got or what you have left. However there was a bug where halfway through the game, an orb would no longer appear if you didn't get it earlier.

Imagine trying to complete all the collectibles in Breath of the Wild, and you are told you can't get one, and you have to start a new game to 100% complete it ;) That's how Crackdown felt.

Final Fantasy 7 Steam Version: Materia Master

We though we were safe from Square. You could miss something, but it wouldn't lock you out of a better ending or missing content, area, boss fight, best weapons, etc. But now the Steam versoin adds an achievement where you have to go out out of your way to remember which fiddly little Materia you can miss. And you thought the Gold Chocobo was tedious!

The true ending in Suikoden 2. You have, maybe, 2 seconds to select a choice, otherwise you don't get it. And it's pretty damn close to the end of the fucking game too.

My bad, that isn't my least favorite, it's just pretty annoying.

Excalibur 2 in FF9 though.

When i learned about this i never fucking lost that chest again, but then again i never actually gotten all the hidden chests either so it was moot lol

Final Fantasy 6: Completely losing a playable character for the remainder of the game if you don't know to do something very un-intuitive right at the end of the World of Balance section.

Cave Story: Best ending is permanently missable if you don't carefully search a room before triggering a cut scene, even though there is nothing indicating that you should do this and there is a time limit encouraging you not to explore.

Terranigma: You cut off many items and much of the best content of the game if you vote for the wrong candidate in Loire. To be fair, it is pretty obvious who you vote for, so this one isn't as obnoxious.

Chrono Trigger: You can miss a playable character unless you refuse to fight him at a certain point, which is very un-intuititve given the way the question is structured and the way the game is leaning.

I have one that's been taunting me for weeks now. Dead Island Riptide Definitive Edition, Dream Team achievement. I swear I played the whole game in co-op with a friend, but it says 89% complete on the progress bar. It's annoying because there's no way of knowing which main story quest I'm missing. I love the DI games, so I'll co-op from the beginning and go all the way through again, but I can't find a partner who doesn't flake out after a few chapters. My friend is a teacher and we were playing at night when they were on strike in Tennessee.

Another one that was annoying, on the first DI game, was Gladiator School achievement. You have to beat 15 consecutive waves in the arena with each of the 4 main characters. If you've got a good friend that is willing to do all the work, it's not difficult, but none of my friends played DI on Xbone, so I had to look in groups, but again, lots of flakey people. So I ended up playing as each character, get them to level 20 or higher, get a few good guns to dupe, buy tons of bleach and metal scrap, get the blueprints for ammo, and do it solo.

I got Gladiator school a couple weeks ago, but Dream Team is still at 89%. Both were the last achievements I needed for those 2 games.

And I nominate Tales of the Abyss. Several amazing easily missable items.

I played through Lost Odyssey twice because I didn't get the achievement for getting all seeds or treasures or whatever. The 2nd playthrough I used a guide and still didn't get it. I can laugh about it now, but I was so put off back then. It was the last achievement I needed too. Lol.

Only one I can remember for sure is the Zodiac Spear. I know there are other games where I really had to think about whether I should restart or not, and I usually do if it's bothering me.

Ha, I hope that was an intentional thing by Matt & Trey. If i recall, one of Trey's favorite games was Earthbound, so he probably played the other SNES RPGs with frustrating skippables.

Trying to do a 3HP run of Cave Story+ far surpassed my frustration with anything else in that game. Never again.

I don't think I minded anything with Chrono Trigger because of the New Game+, which gives you a good reason to restart it for a crapton of extra endings O_o

The "Perfect Crime" in Heavy Rain, where you manipulate the plot to let the Origami Killer win, survive and get away without any clues left behind. Those are the types of achievements I like replaying for, not "collect 1000 macguffins" type stuff.

Definitely I love achievements that force you to play the game a different way, it shows you the devs spent the time to carefully craft different ways to play and encourage you to try them :)

Did they actually make a patch to make skippable Gwent cards no longer skippable? If so, that is mindblowing to think devs would spend the labor for something like that.

I hate RPGs where you get an achievement for doing all the side quests, but they are timed quests and some of them you have to finish others before another time to even unlock it etc. and it's so annoying because if I see just one of them is incomplete I'll get angry and just stop playing it

Beating Original Splinter Cell without killing anyone for the Pacifist trophy. Including winning the scripted shoot out sections by knocking them out non-lethally.

...only to find out the game is bugged to hell and back, and some enemies are glitched and will register as dead as far as the game's internal values go even if you knock them out. There's a guide to follow as to which enemies are glitched and should not be interacted with, but fuck that. Not doing it again.
