our insured has a rail line that goes through her property and she asked if the insured would be responsible for any injuries that happen on the railroad

I don't know, maybe she has authority to prevent them from doing inspections/repairs since it's her land? Doesn't seem that stupid to me but I'm not an expert in railroad maintenance laws

it would be just like a utility meter on your property. that is owned by the utility company and you don't maintain it and have no responsibility over it.

Generally speaking when a public utility like a railway has passage through your land part of the agreement that you have to sign as the landowner allows them access for maintenance, inspections, and repairs. You are not allowed to deny them access, though they must compensate you for any damage they cause as part of their maintenance - for instance, if you are a farmer and a pipeline is buried in one of your fields, the operating company has to pay you for any crops they have to remove when excavating the pipe (generally speaking they also re-seed the soil when they backfill, so you basically get paid twice).
