For the Eternal Mangekyo or whatever. The wiki says they needed the eyes of someone close to them to restore their vision, so why couldn't they just trade eyes? Guys have survived much more serious bodily harm in the show haven't they?

I can't seem to find an explanation for that and it's just bugging me for some reason.

They also need strong emotions to trigger the sharingan's abilities. Agreeing beforehand wouldn't work.

Do those emotions need to be negative? Why can't they be all "I love you so much bro, I'll even give you my eyes!". And the other guy would be "Nah I love you even more, take my eyes!"

Because having the mangekyo is a rare ability. Most uchiha probably didn't have the sharingan only a few possessed the mangekyo.
