I was super tired making my Wizard in Diablo 3 a few weeks ago. Was like 2am. Spent a couple hours playing, made some decent progress.

Got on the next day and remembered I named him "Balls"

FTL (like any good roguelike) gives you lots of tough choices. Do I go the high risk route that gives better loot and possibly a chance at a better ending? Or play it safe and hope I get more chances or good equipment later on?

It's tempting to go high risk every time but you'll probably never win if you do that.

whether to put the last Sasquatch out of the misery i unknowingly and carelessly created for it or to spare it and leave it to grieve its loved ones and know it's the last of its kind

i don't even remember what i chose to do. i just remember i stopped playing the game after that cuz it was the first time actions taken in a game made me personally feel like a complete douchebag.

Oh fuck I forgot about that

Now I'm sad

In recent years, the one that sticks out is from Witcher 3's Hearts of Stone. It's whether or not take the rose from Idris von Everec.. There are no consequences, they just do such a wonderful job characterizing her that it makes the choice carry a lot of weight.

Ragnarok the Sword or Ragnarok the Esper?

Yes I know the Sword is the better choice and Ultima can be learned from the Paladin Shield so you literally don't even need the Esper, but not taking the Esper really hurt my OCD at the time since it's the only Esper I wouldn't have. Thankfully, there is another way to get the sword in the newer versions.

I know between NV and Oblivion there were some amazing ones but I can't remember off the top of my head.

Maybe anything where you had the power to cause a big explosion. On one hand that might make factions made at me and ruin the gameworld, on the other hand boom booms are fun.

Witcher 3 feels like they really put effort into your decision. Just the Baron alone with him being an ass, then feel sorry, then an ass again, then sorry.
