Do you think they really have a concept of clothing? Or do they just assume it's like our fur and we have really strange camouflage abilities, or that we're all shape changers that shed our skin constantly?

You can train mice to push buttons for food. That doesn't prove they know what how a button works. They just now "if I bump into this red thing I get food." Your dog associated your shoes with going for a walk because you never went outside without them on. Of course a dog isn't going to know or care where a shoe comes from or what the mechanics of it are. But your initial question was whether they understood that it wasn't a part of the hum an and the answer to that is yes.
