Depends what type of family you're referring to. I mean if it's parents being abusive for example that's worse as they are the ones who created said person and should be the ones who brought said person up, provide for them and kept them safe. A lot more trust is put into family. To have that trust broken isn't something that's easy to move on from.

Where as in a abusive relationship with a person someone has grown to love, while still not easy to move on from these people generally haven't been part of their whole life and can be replaced eventually. I know cases differ from person to person and this is a very broad question but I'm simply going on the average case. I guess the short answer is the amount of trust put into someone is the difference.

"Abusive" describes certain varieties of harmful behaviour within the relationship, while "family" describes the level of consanguinity. They're largely causally unrelated terms, though there can certainly be overlap.
