I don't think he's ever made one from one planet to another. Like I've only seen Earth portals to other places on Earth, and he's only portaled on Titan from one place to another. No possible way that Strange could have viewed realities that included future events that included Thanos rewinding time in a million and change possibilities. He had enough willpower where he would've immediately broken free of whatever Mantis was doing and reclaimed it. And if not, they're still on Titan with no way home.
