For Fate/Zero, I think it broke down like this:

The Einzberns gave Kiritsugu Avalon to use a catalyst to get Altria/Arturia/Saber.
Tokiomi grabbed something for Gilgamesh. They show it in the first ep but I don't recall off-hand.
Kayneth originally had a piece of Alexander the Great's cloak but Waver stole it to summon him. Diarmuid was a last minute backup he had to use since his planned Servant was stolen from him.
I don't think we're told what the Matous used for Lancelot but presumably they had something.
Gilles/Caster I think was the one example of the Grail just picking one it found most suitable to Ryunnosuke when he performed the ritual for lolz.

Assassin is... a case where the "rules" used to state that all Assassins summoned had to be one of the Old Men of the Mountain/Hassan-i-Sabbah because by definition a successful assassin would perform their job so well they couldn't be famous enough to count as a Heroic Spirit and the Old Men of the Mountain were the exception to the rule. This rule has since been thrown out the window in later media.

And of course both in the backstory and current media you just straight up can break the rules if you know what you're doing/plot convenience. Like the Einzberns (who were involved in creating the system to begin with) summoning an Avenger in the War before Fate/Zero (which had... consequences) or F/SN's Assassin situation, or countless others.
