I don't believe it's a formal legal requirement (not in the US, anyway), but it is fairly common for doctors to refuse to perform the procedure on a woman without her husband's permission. While I haven't confirmed this, that most likely means some guy has successfully sued a doctor for giving his wife a hysterectomy without his permission (since avoiding malpractice suits is a very common motivator for doctors refusing procedures they have no obvious reason to refuse), which in turn means there's legal precedent for encouraging doctors to get that permission. --- This is my signature.
