I'm honestly not sure if it is sovereign beyond a certain point. Keep in mind that only a certain number of miles from a nation's coast are considered sovereign and I half assume that the same principle applies to air space.

Wiki'd it and I seem to be correct in that assumption, although it's a little unclear how far up that extends

Not really. Half the world has at least one country separating it from its intended target. However, it's not declaring war on all of those countries in-between every time a missile is fired.
