Proven habitual medication nonadherence or insurmountable psychosocial barriers to posttransplant adherence are a relative contraindication to transplantation. Patients with alcohol and/or substance abuse or those noncompliant with dialysis treatment need to be rehabilitated prior to transplantation. We require that such patients enroll in a rehabilitation program and remain drug or alcohol free, confirmed by random drug testing, and compliant with the dialysis treatment for at least one year before becoming active on the transplant list. Appropriate transplant recipients should be immunized against influenza, pneumococcus, hepatitis B, and varicella, if antibody negative. Vaccination for Haemophilus influenzae and meningococcus should be considered in asplenic individuals and those who may require therapy with eculizumab. (See "Immunizations in solid organ transplant candidates and recipients".) --- |

Hospitalists and infectious diseased trained physicians carry administration roles as well, (although I am only night hospitalist, so my role doesn't involve administration) are also involved in making some policies in regards to vaccines. I just saw a 23 year old who didn't take the vaccine because he was waiting for FDA approval is now intubated and on ECMO. Do you guys really want to be like that? --- |
