Can assume a current argatroban dosing rate is less than 2 mcg/kg/min.Never encountered this situation before, would probably curbside hematology however, it seems like the half life of argatroban is 1-3 hours, I'd probably just D/C it and start warfarin to get to therapeutic INR (making sure INR wasn't elevated in the first place, though). --- |
