A lot of sick people and only so many physicians to go around.

Right.

Even the ones make sense are sometimes too much. Like, Cleopatra likes civs with large militaries and hates civs that don't. So, if you encounter her early, when you only have one or two cities and a handful of units, she can turn hostile very quickly.

Other than the Agenda system, I do think the game is overall better than 5.
