You can try breaking large tasks into smaller tasks with individual deadlines.
It can help too to figure out what time of the day you're most productive and schedule time to work on things then.
You can recruit someone to hold you accountable, like ask them on a particular day to ask you if you did the thing you had resolved to do. Knowing they're going to ask can help motivate you.
If you keep avoiding a thing it's also sometimes worth asking if it's something you really want to accomplish or just something you've been made to feel you "should" do. Is it something you really need to do in order to reach the goals you want to reach?

Make at least the first couple of steps as easy and fast as possible to do in your head.
"I will open my computer"
"I will log in"
"I will open my document"
"I will type 5 words", etc...
Helps trick the goal setting part of your brain into feeling pretty good and motivated by itself
