Having watched a ton of Plainly Difficults industrial accident videos, Im going to guess it was a mixture of shoddy maintenance combined with good old fashioned complacency that likely caused this, as thats been the bane of many other industrial plants over the years.

I was getting like that over the years sadly until I realized all that I was doing was making myself miserable and nobody wanted to be around me as a result, thus I made the conscious effort to move away from the culture war nonsense and focus on what made me happy instead.
