Probably putting something in the wrong account. Or rounding error. Speaking as an accountant, there are some legitimate ways an error like this can occur.

I found an article that said something like they used the wrong pricing for things. They used the value of replacing an item instead of the value of the item. I'm not an accountant so I don't know exactly what that means.

It's because a lot of the equipment they're sending Ukraine has just been stuff the military had stockpiled. So they were using the price of something new when they should have been using the price of something that's been sitting in a warehouse for X amount of years.

So, it was like this:

"We sent equipment worth 10 Billions of dollars to the Ukraine!"
"Sir, it was used stuff, it was only worth 3.8 billion dollars."
"We are going to send another 6.2 billion dollars to Ukraine!"

Yeah, basically. The headline is sort of lying, it's more like we sent them 6.2 billion less than we were supposed to.

Yeah the headline is misleading, the "provides extra $6.2 billion" is more like they had a specific budget, and just now realized they have not gotten through as much of it as they thought they would so they have another $6.2 billion still to spend. The actual amount didn't go up, the portion used just went down

Not like we would have done anything other than give it to rich people here anyways. At least this way it goes to a worthy cause.

That would do it too. Items do depreciate over time and that reflects in it's resale value to a company's books.
