Obviously, companies will invest more in the infrastructure of a region with higher density of population (because more customers.. aka urban settings) than less densely populated areas (aka rural).
My parents are lucky enough to live within range of a 4G cell tower. If wireless wasn't an option they'd be limited to 56k dialup because their phone lines are so old they'd have to be ripped out and replaced to even have medium-speed DSL service. The phone company estimated it would cost about a quarter million dollars to give wired service to about 10 homes in their area in 2010, so probably half a million now.
