It goes back to the Second World War. The government has instituted wage caps, and employers starting offering health insurance in lieu of higher pay.

here's the important part:

First, during World War II the War Labor Board ruled that wage and price controls did not apply to fringe benefits such as health insurance, leading many employers to institute ESI. Second, in the late 1940s the National Labor Relations Board ruled that health insurance and other employee benefit plans were subject to collective bargaining. Third, in 1954 the Internal Revenue Service decreed that health insurance premiums paid by employers were exempt from income taxation.

I just got back from the ophthalmologist. Got 2 pairs of glasses. I knew it was gong to be expensive, but fuck, it was a lot more than I was expecting, even with insurance.

Yes and then postwar they realized that if people's healthcare remained tied to their job then they had a major level to use against their employees to help keep them from getting uppity.
