Back then, R ratings were more based on vulgar language than suggestive content, nudity, or violence.

Jaws was rated PG, and there's a ton of blood, severed body parts, and a man literally being eaten to death.

Brief nudity could sometimes skirt under an 'R' rating in the days before PG-13 - Sixteen Candles is another example off the top of my head. Later, Titanic managed to stay PG-13 despite the nudity.

I'm guessing these examples managed to wiggle under an R rating by keeping the nudity non-sexual - the nudity in Airplane! is pure absurdist comedy, Sixteen Candles just has Molly Ringwald gawking jealously at the popular girl while she's in the shower, and Titanic probably got away with the nudity being artistic in nature.

A topless woman runs past the camera in one scene. She's only on-screen for about a second or two.

Anyways, PG-13 didn't exist when Airplane! came out. So the choices were either PG or R, and Airplane! just wasn't risque enough to warrant an R rating.

PG movies could typically get away with a single f-bomb in the pre-PG13 era. Spaceballs is rated PG.

The reason is that up until the early 2000s you could briefly show nudity in PG-13 movies without it going to R, and the same rule applied to PG movies before the PG-13 rating existed. Look at Hackers or The Fifth Element; we briefly get to see Angelina Jolie and Mila Jovovich's boobs in those.
