I know everyone tends to say Thessia, and sure enough, it does have you losing in a cutscene against the absolute worst character in the series, and it's the peak of Mass Effect 3's forced melodrama. But if you bring Javik and Liara you get some great dialogue and new lore, the latter of which is especially missing from the rest of 3.

I actually think the Palaven Moon at the start of 3 is a contender. "Generic moon" environment, nothing interesting dialogue/story/lore wise, and a fucking turret section in 2012, in what was previously mostly an RPG series. It feels like a Gears of War level, and not a good one either.

Kai Leng once stole Cpt. Anderson's cereal. You have any idea what kind of a stone cold badass you need to be to pull something like that off? He is a credible and serious threat.

If you fuck up in a specific way he instead kills Kirrahe, which does nothing to engender goodwill.
