Aside from obvious ones like current monarchies. Or I'll try and find some famous person from Ancient Rome and see how far ahead I can trace their descendants into the Middle Ages.

Well one example is that Emperor Theodosius who lived in the mid 300s had recorded descendants until after Justinian who lived 200 years later.

One thing I thought was cool was that a lot of early Frankish monarchs like the Merovingians have relations to Gallo-Roman officers and politicians in the 300s and 400s. It seems like as Gaul become more untenable from Rome, there was a greater diaspora of culture.
