I think the worst was the one in Mass Effect 1

It would just pause the screen and make press the highlighted controller buttons. No problem solving. No difficulty. Doesn't even make you feel like you're hacking anything. It's just a time waster and nothing else.

Shadowrun on sega genesis was good for its time. Not proper hacking though its basically its own version of going into the matrix (even called the matrix) to fight anti hacking systems and steal information to sell on the black market.
