It was a 4chan thing, it was parodying how every name in MGSV is First Name "Code Name" Last Name. Like Revolver "Shalashaska" Ocelot or Punished "Venom" Snake. They'd take Kaz's name and go Kazuhira "[racist joke]" Miller. Saying things like if Snake fultons an African-American soldier that Kaz wouldn't let them onto the base for example.
