The Cannonball Loop was probably the most infamous attraction at the already infamous Action Park in New Jersey. It was an enclosed water slide with a complete vertical loop near the end. During testing one of the dummies sent down the slide came out decapitated. Employees were offered $100 each to test it. The first one to try it broke his nose in the loop.

Riders were weighed before going down the slide: too heavy and they'd go too fast and be slammed into the wall of the loop causing broken noses and lost teeth, too light and they wouldn't pick up enough speed to clear the loop and would get stuck. A hatch had to be installed in the loop to allow rescues.

The ride was only open for a month in 1985 before it was shut down due to injuries. It opened again for a few days in 1995 and 1996 before injuries closed it permanently.
