could have been people who are there on a hold for self harm, or talking about self harm. they're just kinda forced to lay there with access to nothing and a nurse constantly watching them so they don't hurt themselves...a lot of them will act out or scream. if the Nurse in question didn't seem nervous she probably works near that department, or the area where they hold those people.

It's the cardiac area so maybe he was ripping his heart out and couldn't figure out the rib cage
