California is just two weeks into its long-awaited reopening, but already a new coronavirus threat has prompted Los Angeles County health officials to request a voluntary rollback of one of the freedoms fully vaccinated people only recently began to enjoy.

The countys recommendation this week that everyone regardless of inoculation status should wear face coverings in public indoor settings as a precaution, given the presence of the worrisome Delta variant of the coronavirus, underscores that speed bumps may still lie ahead on the road to pre-pandemic normalcy.

It also illustrates the current landscape in the long-running battle against COVID-19: one where those who have had their shots may be asked to give a little to help shield those who havent.

Theres a lot of give-and-take in our communities, L.A. County Public Health Director Barbara Ferrer said during an interview Tuesday. And this is the time for vaccinated people to give, I think, and protect others.

If theres one lesson weve learned over the last 18 months its that, with a new virus, there are lots of unknowns, Ferrer said. And every time we get a new variant of concern thats identified, it takes some time to really understand whos getting infected and how easily theyre getting infected and what are the consequences of that infection.

Of particular concern, she added, is that theres a lot of opportunity here, because we have large numbers of unvaccinated among our workers, to have large outbreaks that really can once again wreak havoc in families and communities.

We have the tools to prevent that from happening, Ferrer said.

The U.S. Centers for Disease Control and Prevention continues to advise that those who are fully vaccinated dont need to wear masks in most situations, given the protection the shots provide including against circulating variants.

The World Health Organization, on the other hand, says people should wear a mask, especially in crowded, closed and poorly ventilated settings, even after theyve been vaccinated.

I think the first message we want to be careful about is saying, Once youre vaccinated, you can just go ahead and do whatever, Dr. Bruce Aylward, senior adviser to the director-general at the WHO, said during a recent briefing. Yes, you can reduce some measures, and different countries have different recommendations in that regard, but theres still the need for caution.

He added: We still live in a world that is only partially vaccinated, that has a lot of susceptibility, a lot of vulnerability. So, what were saying is, once youve been fully vaccinated, continue to play it safe because you could end up as part of a transmission chain.

Caution is needed.

Yeah Im still wearing my mask indoors. I went to pick up a prescription yesterday and most people dropped theirs. Most businesses are still requiring them but I dont think theyre really enforcing it. I rarely go anywhere indoors though so its nbd, today Im meeting a friend for lunch but were having it outside.

Nothing we do is going to get these people to be vaccinated; least of all us masking up.

I will save 100% of my concern for their soon to be orphaned children who are to young to have been fully vaccinated. Maybe their new parents will take better care of them.

Thankfully it seems like a good amount of people in LA are still being cautious. I'm still seeing most people masking up and keeping their distance when I go out for lunch. And when we went to Universal Studios on the first Saturday after California "fully opened", I'd estimate at least 50% of the people there were still wearing masks everywhere, except for the dining areas.

As state and local officials drop mandates on COVID-19 restrictions, the virus continues to circulate throughout Florida, sending increasingly younger people to the hospital at rates that are among the highest in the country.

Overall, the number of people being treated for COVID in Florida hospitals has ticked down over the last two weeks after a slight rise in the beginning of April. About 3,000 Floridians were hospitalized with COVID on Thursday, down about 10% from 3,345 patients on April 23.

But Florida ranks third in the nation for confirmed recent COVID hospital admissions for adults on a per-capita basis, behind only Michigan and Kentucky, according to new federal data analyzed by University of South Florida epidemiologist Jason Salemi. The admissions are concentrated in patients in the working-age bracket between 30 and 50 years old.

That comes as the rate of hospital admissions for older age groups declines, likely because older people are more likely to be fully vaccinated, Salemi and other experts agreed.

The younger skew for hospitalized COVID patients also follows an earlier rise in COVID cases for working-age people in late March. Public health experts tied that rise in infections to Spring Break tourism.

When you have an increases in cases, you tend to have an increase in severe illness, Salemi said.

More than a quarter of confirmed COVID hospital admissions in Florida for the week predating May 1 were among people between 30 and 50 years old, compared to 13% in the week predating Jan. 15.

Behind Florida are Delaware, Pennsylvania, Maryland, Georgia, New Jersey, Connecticut and Colorado.

Floridas high hospitalization rate for the younger age brackets matches what Dr. David De La Zerda has been seeing at Miami-Dades public hospital, Jackson Health System. The intensive care unit leader and pulmonologist said COVID patients are averaging between 30 and 50 years old, many of them healthy and without conditions such as obesity or diabetes.

I think its because theyre the last group to get the vaccine, De La Zerda said on Thursday. This age group feels they dont need a vaccine because theyre not going to die from COVID.

But De La Zerda said even if they are less likely to die from the disease than those older than 65, the recovery can take three to four months before the younger patients even start to feel better, and then they still have lasting effects.

We have a bunch of post-COVID patients and theyre young but their lungs look like a pulmonary fibrosis, De La Zerda said. We have to start looking at transplants.
