I've used Instacart (for Kroger, Sprouts, and H Mart orders) about ten times, in the pandemic and the only real problem I had was with some confusion with produce amount. One time, I ordered 1lb of jalapenos and got one loose jalapeno. Another time, I ordered 4 large bananas and got 4 bunches of large bananas. I didn't get a text or call asking for clarification, either. Wasn't a big deal but was annoying. The produce has always been about as fresh as I could ever find for myself. Substitutions are also very iffy, at least from what I've seen with other people.
