People with PhDs and such typically don't put so much emphasis on it. They'll mention it like they mention their name, but that's it. As far as I know, Jill Biden has not made any fuss about being called Dr. This guy just decided to create an issue where there was none due to an inferiority complex. As a PhD holder myself, I tell my students in my statistics classes my full name, and that they can call me Dr. Lastname or call me by my first name. Doesn't matter to me. Still, 99% of students will refer to me by my Dr. title or simply "Professor" out of respect. With that being said, I do know that many women and minorities do insist that they be acknowledged as Dr. simply because their education and expertise is constantly questioned and demeaned, so I think they are justified in that.
