Theres often low visibility, tight spaces, unexpected and fast currents... all potentially deadly on their own. Its very easy to get lost or trapped and have no way to get to safety. From a quick google search, looks like the primary issue is getting lost. Cavern divers use a special style of flutter kick to avoid stirring up the sand to the point that it would reduce visibility. They also leave a line behind them so they can follow it back the way they came. Apparently theres also an issue of normal back-mounted oxygen tanks being in weird position to fit through places so cavern divers use side-mounted ones but thats probably more of a comfort thing. Anyway, yeah. Being trapped in an underwater cave system sounds like a special kind of hell.
